Trusted Assessors: Presentation & Webinars

7 August 2017

Through the use of a Trusted Assessor, we can reduce the numbers and waiting times of people awaiting discharge from hospital and help them to move from hospital back home or to another setting speedily, effectively and safely.

This presentation describes how local systems could implement trusted assessment to reduce, effectively and safely, the numbers and waiting times of people awaiting discharge from hospital and is also a key part of the High Impact Change Model, as set out in the NHS Five Year Forward View Next Steps document. The Developing trusted assessment schemes: ‘essential elements’ guide has been jointly developed by; NHS England, NHS Improvement, the Local Government Association, the Association of Directors of Adult Social Services, the Care Provider Alliance and the Department of Health.

Many people wait too long for discharge from hospital resulting in poor experience of the health and care system and poorer outcomes.

Trusted assessment is a key element of best practice in reducing delays to transfers of care (DTOCs) between hospital and home.

This guide describes how hospitals, primary and community care and local councils could work together to deliver trusted assessment as a key part of the High Impact Change Model (HICM).   Implementation of the HICM is a national condition for funding through the 2017-19 Integration and Better Care Fund policy framework.

This is an example of a series of resources that NHS Improvement is developing with partners to help you rapidly meet expectations on urgent and emergency care, as set out in the NHS Five Year Forward View Next Steps document
